Download Advance Directive, including Addendum A
The language of the Advance Directive was established by the Oregon Legislature. This form is helpful, but not required by the state. Addendum A is not an official form, but does clearly state the wishes of many Christian Scientists. Both forms make one’s care wishes clear to family, friends, or caregivers should one not be able to communicate his or her desires at the time.
- Advance directives inform first responders and caregivers of your desires for help if you are unable to.
- They authorize a healthcare representative to advocate for your desires.
- Addendum A specifies your desire to receive Christian Science care exclusively.
- A completed advanced directive is required by Canterbury in order to provide continuing care.
